What is the OMNIA Health Alliance?
Through the OMNIASM Health Alliance, RWJBarnabas Health Partners collaborates with Horizon Blue Cross Blue Shield of New Jersey to change the way health care is delivered in New Jersey. We are pleased to be working with Horizon BCBSNJ to identify opportunities to improve and better coordinate health care for our patients. The focus of the OMNIA Health Alliance is to help keep individuals healthy through increased population health management and more integrated, coordinated care that rewards better health outcomes, an enhanced patient experience and lower cost care.
The OMNIA Health Alliance consists of certain doctors and hospitals across the state that share our commitment to transforming health care in New Jersey. Working together, we are focused on driving up quality care and driving down costs for the people of New Jersey.
The OMNIA Health Alliance aims to transform health care in New Jersey by providing more organized, personalized and coordinated care, powered by the secure sharing of information, processes and best practices to help patients get and stay healthy.
